Community Care

Commonwealth Home Support Programme (CHSP) is a basic level of home services that provide eligible people with help at home, support in getting out into the community, and a break for carers. to be eligible for these services clients must have a RAS assessment – referrals are made via My Aged Care.

Commonwealth Home Care Packages (HCP) are designed to suit each client’s individual needs. To be eligible for these services clients must have an ACAT assessment – referral are made via My Aged Care.

Level 1 and Level 2 support people with low care needs.
Level 3 and Level 4 support people with complex care needs.
