Extended Aged Care at Home (EACH)

The EACH program provides services that support clients with high or complex care needs to remain in their own home. To be eligible for these services clients must have an Aged Care Assessment Team (ACAT) assessment. OzPol can arrange referrals to ACAT.

Individual packages of care are designed to suit each client and may include:

Nursing care
Medication assistance
Hygiene assistance
Domestic assistance
Respite care
Social support
Meal preparation
Transport
Shopping
