Community Aged Care Packages

For many people living in the comfort of their own home is an important part of growing older, but for some older people this may became difficult without extra help.
This is where Community Aged Care Packages can assist. Community Aged Care Packages focus on allowing older people to remain in their homes. This ensures minimal disruption to their life and may prevent early entry into a residential care facility.
To get help from the government, an individual must undergo an Aged Care Assessment by an Aged Care Assessment Team (ACAT) to establish which services are needed so that physical and/or psychological independence can be maintained. OzPol can arrange referrals to ACAT.
This case managed service takes into account the lifestyle, cultural background, and religious beliefs and can include a possible combination of the following:
